What we collect

When you use the “Find my next step” questions, we collect:

• Your answers: who the questions are for, where you are in your recovery, and how soon surgery is, if it is scheduled.
• Your first and last name, email address and phone number.
• The page you were on and how you reached us, such as the website or campaign that sent you.

Please do not put medical records or detailed health information in the form. We will talk about your situation with you directly.

How we use it

• To follow up with you by email about your next step.
• To understand which pages and campaigns bring people to us.

We do not sell your information.

Who else handles it

We use service providers to run the site and keep your details: our customer management system (HighLevel), our website host (Vercel), Google Analytics, which measures visits to the site, and Google Fonts, which serves the fonts on this page. They handle your information only to provide those services. We also share information when the law requires it.

Cookies and tracking

We use Google Analytics to understand how people find and use this site, such as which pages are visited and how visitors arrived. Google Analytics uses cookies and collects information like your device, browser and general location. It does not receive the answers or contact details you send us. You can opt out with Google’s browser add-on at tools.google.com/dlpage/gaoptout. We do not use advertising trackers.

Children

This site is for athletes, parents and clinicians. If you are under 18, please ask a parent or guardian to send the form for you.

Your choices

You can ask us to show you, correct or delete the information we hold about you by emailing evan@eliteacl.com. You can stop emails at any time with the unsubscribe link in them.

Changes

If we change this policy, we will post the new version here and update the date above.
